  • Physical Examination Certificate signed and stamped by Medical Officer (Physical Examination Form China)
  • English proficiency certificate issued by previous University (IELTS, If available)
  • Recommendation letters (2) issued by Professor or Associate Professor of recent college/university
  • Research Proposal & Study Plan
  • Experience Certificates (If available)
